Various documents are required for the application for parental allowance.
- The salary statements for the last 12 months
- This is provided by the Human Resources Department on request.
The following additional documents are also required:
- Parental benefit application
- Optional: supplementary sheet for self-employed persons
- Other evidence and certificates
- Original birth certificate with the purpose "parental benefit" or "social purposes".
- Name, address and membership number of your health insurance
- For non-EU foreigners: passport copy incl. residence title or certificate from the foreigners authority
- For single parents: Current salary statement with tax class II or proof from the tax office that you meet the requirements for the relief amount
- For the partnership bonus: Proof of part-time work arrangements of the employer for both parents (as soon as available)
- Adoptive child / adoptive care: Decision of the court of adoption or confirmation of the youth welfare office / adoption agency
- Tax assessment of the year before the birth of the child
Proof if you have not made compulsory contributions to pension insurance, health insurance and long-term care insurance
List of the number and periods of child allowances if you have not received them over the entire investigation period
